Why do migraines happen around the time of a woman's period?

To answer that question we need to talk about hormones, and in order to do that we must start with the brain. There is an area of the brain called the hypothalamus that, together with the pituitary gland adjacent to it, controls the hormones necessary for human reproduction as well as many of the periodic functions of the body.

Glands secrete hormones as "micro-messengers" into the blood stream to provide instructions to the body. These micro-messenger hormones find their way to the specific cells that can read the messages and act on them. These cells are called "hormone receptors."

The menstrual cycle is controlled mainly by two micro-messenger hormones secreted by the pituitary gland. The two hormones are the luteinizing hormone (LH) and the follicle-stimulating hormone (FSH).

Selective hormone receptors found in the organs of the reproductive system receive these hormone messengers and respond accordingly by producing either estrogen or progesterone, depending on which type of hormone messenger they received. This is all part of the natural cycle of a woman.

In the bodies of women and adolescent girls who suffer from migraines related to their menstrual cycle the normal function of the hormone receptors can become unbalanced or out of sync with the production of female hormones. When this happens, the hormone receptors send neurochemicals into the blood stream. Once in the blood stream they quickly travel to the very ends of blood vessels that surround the tiny fibers of the trigeminal nerve located on both sides of the head.

This neurochemical actually oozes through the thin walls of the vessels and comes in contact with nerve fibers. The fibers are irritated by this body chemical and react to counteract this irritation by excreting a contra-chemical that in turn inflames the blood vessels and makes them swell.

In a tit for tat exchange more and more blood vessels begin to swell to the point where they literally squeeze or pinch the nerve fibers and you feel it as the severe pain of a migraine headache.
